Social prescribing: coming of age
Tuesday 6 November 2018
9am-5pm
One Day Seminar/Conference
Following our sell-out event last year, this year we will be moving from examining the case for social prescribing and the benefits it offers to exploring the ways in which it can be embedded and implemented. In collaboration with the College of Medicine and the Social Prescribing Network, we will look at how it can be measured and the impact it is already having on outcomes for patients.
Come along to this event to hear from local areas around the country who have implemented social prescribing models and can report on their learning and outcomes and provide practical resources for commissioners, clinicians and voluntary and community organisations looking to adopt and implement social prescribing in their own locality.
At this event you will hear about:
- the progress of social prescribing in England, Scotland, Wales and Northern Ireland
- how a social prescription model can be adopted and implemented within a place-based health and care system
- how to embed quality assurance within social prescribing models
- the evidence so far on impact and outcomes resulting from social prescribing interventions
- the role of technology and big data in supporting the delivery and analysis of social prescription
- the ways in which the public, voluntary and community sectors are working together in new ways to collaborate over delivery of social prescription locally.
